Marked Reduction in HbA1c With Personalized Diabetes Coaching

A 65-year-old female drops her HbA1c by 3.1% through CGMS-based lifestyle intervention program with personalised coaching without any change in her previous medication dose in just three months. When she joined the program, her HbA1c was 10.8%. By the end of 3 months, she had reduced it to 7.7% – which is incredible!!

Case Presentation

A 65-year-old female was living with type 2 diabetes for 30 years. She weighed 54 kgs, was 152 cm tall and had a BMI of 23.46 kg/m2.

Although she was a little overweight but was energetic otherwise. She was physically active and went for daily walks of 3 km.

One of the major concerns for her was her episodes of low blood sugar (hypoglycemia). This was extremely worrisome for her as she lived alone. Her blood sugar got low more frequently in the night. She also had high blood pressure for 4 years and was taking medicines for the same.

The patient was referred by an endocrinologist and enrolled in an integrated AGM-based lifestyle management wellness program by Fitterfly – DiabeflyTM

Problem areas identified with its solution

Patient’s DiabeflyTM Coach helped her achieve these results by identifying her problem areas and providing a solution for them.

Here’s what they worked on:

1. Extremely low blood sugars (Hypoglycemia)

She was experiencing alarmingly low blood sugars at midnight and early morning. Her Coach suggested having a bedtime snack like milk and also advised on portion monitoring for meals especially for dinner. Click here to read more about the tips to prevent and deal with hypoglycemia.

2. Insufficient Dinner

As she stayed alone she was not much into a full meal and prefered one meal dish. This did not meet her nutritional needs, particularly of protein.

Keeping in mind that the patient was a vegetarian, her Coach suggested vegetarian protein-rich meal options and their combinations. So that the Recommended Daily Allowance (RDA) of carbohydrates, fats, proteins and other micronutrients is met.

3. Consumption of stale food and lack of variety in meals

Since she lived alone she was less inclined to cook multiple times in a day. So she ate the same meals throughout the day which were lacking in variety.

She often consumed stale food as well. Her coach advised her on healthy food storage techniques – how to refrigerate, how to reheat, keeping nutrients intact and much more. She was also advised to cook limited portions only whenever possible.

4. Elevated postprandial blood glucose levels

The patient did fasting occasionally and preferred having sago when she broke her fast. It raised her postprandial blood glucose levels beyond the target range. Her Coach guided her on meal options for fast and the quantity for the same.

5. Consumption of processed packaged snacks in the evening

Since the patient was less inclined to cook fresh for herself, she was largely dependent on processed snacks easily available at her local grocery store. To name a few – chakli, bakarwadi and many more.

These fried unhealthy snacks with a high glycemic index (GI) caused huge glycemic variability and poor health outcomes. Her coach guided her on protein-based snacks like roasted chana etc. for the evening.

Results from the DiabeflyTM program

Changes in blood sugar levels while on the CGMS device for the first 14 days of the program.

  1. Initial results of the first 14 days – week 1 vs week 2.
    • 30% drop in Average Blood Sugar reading from 174mg/dl (week 1) to 122mg/dl (week 2).
    • 14% increase in Time-in-Range (TIR) from 47% (week 1) to 54% (week 2). In other words, blood glucose levels were in a target range for a longer time within one week of the program. Click here to read more about Time-in-range.
    • 62% decrease in Time-above-Range (TAR) from 45% (week 1) to 17% (week 2).
  2. Results at the end of 3 months:
    • Marked reduction of 3.1% in HbA1c levels from 10.8% to 7.7%.

Summary

This case study supports the hypothesis that AGM-based lifestyle modification plan has positive diabetes-related outcomes. It provides the rationale for a larger study with a bigger sample size to further augment this hypothesis.
