{"id":10060,"date":"2023-06-29T10:45:27","date_gmt":"2023-06-29T05:15:27","guid":{"rendered":"https:\/\/www.fitterfly.com\/blog\/?p=10060"},"modified":"2023-11-29T20:00:28","modified_gmt":"2023-11-29T14:30:28","slug":"exploring-the-impact-of-mental-health-on-diabetes-management","status":"publish","type":"post","link":"https:\/\/www.fitterfly.com\/blog\/exploring-the-impact-of-mental-health-on-diabetes-management\/","title":{"rendered":"Exploring the Impact of Mental Health on Diabetes Management"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">India is inching towards becoming the Diabetic Capital of the world, with more than 101 million of its population living with diabetes while an additional 136 million live with prediabetes, as per a recent ICMR study titled<\/span> <a href=\"https:\/\/www.thelancet.com\/journals\/landia\/article\/PIIS2213-8587(23)00119-5\/fulltext#articleInformation\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Metabolic non-communicable disease health report of India: the ICMR-INDIAB national cross-sectional study<\/span><\/a><span style=\"font-weight: 400;\"> (The Lancet Diabetes &amp; Endocrinology, Published: June 07, 2023).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Diabetes is also the leading cause of blindness, kidney failure, amputations, and a major risk factor for heart disease, stroke, and cancer. In this alarming situation, leading physicians recommend holistic care along with early psychological support in diabetes management to prevent long-term complications.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Even the American Diabetes Association and the European Association for the Study of Diabetes recommend focussing on the social determinants of health and physical activity behaviours under the holistic approach to diabetes management<\/span> <a href=\"https:\/\/diabetesjournals.org\/care\/article\/45\/11\/2753\/147671\/Management-of-Hyperglycemia-in-Type-2-Diabetes\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">(Diabetes Care 2022;45(11):2753\u20132786).<\/span><\/a><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<h2><b>Mental Health Challenges Associated with Diabetes<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Diabetes being a chronic metabolic disorder, affects the physical, social and mental health of the patient. Psychological challenges associated with diabetes management include anxiety, lowered mood with impaired ability to self-care, poor eating habits and the fear of developing diabetes-related complications. These mental health issues in diabetes may eventually develop into poor quality of life (QoL), reduced general well-being and depression.<\/span><\/p>\n<h2><b>Bidirectional Relationship between Mental Health and Diabetes<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Multiple lines of<\/span> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5975983\/#:~:text=Diabetes%20patients%20with%20depression%20have,having%20an%20increased%20mortality%20rate.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">evidence<\/span><\/a><span style=\"font-weight: 400;\"> point towards a bidirectional relationship between mental health and diabetes.<\/span> <a href=\"https:\/\/diabetesjournals.org\/care\/article\/24\/6\/1069\/22483\/The-Prevalence-of-Comorbid-Depression-in-Adults\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Anderson et al.<\/span><\/a><span style=\"font-weight: 400;\"> reviewed 39 studies with a sample size of 20,218 subjects to come to the conclusion that there is two times more chance of developing depression among people living with diabetes than non-diabetic individuals in the control group. The study further suggested that diabetic women (28% probability) are more prone to experience depressive symptoms than diabetic men (18% probability).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Even CDC\u2019s <\/span><a href=\"https:\/\/www.cdc.gov\/diabetes\/managing\/mental-health.html#:~:text=People%20with%20diabetes%20are%2020,sometimes%20both%20together%20works%20best.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">report<\/span><\/a><span style=\"font-weight: 400;\"> on Diabetes points out that mental health and diabetes are closely associated.<\/span> <span style=\"font-weight: 400;\">Diabetic patients have 20% more chances of developing anxiety.<\/span> <a href=\"https:\/\/www.mhanational.org\/diabetes-and-mental-health\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Mental Health America<\/span><\/a><span style=\"font-weight: 400;\"> also suggested that 33-50% of people living with diabetes are likely to experience diabetes distress at some point in life. Similarly, people suffering from depression have an increased risk of developing diabetes.<\/span><\/p>\n<h2><b>How does mental health impact the clinical presentation of diabetes?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">People living with diabetes often experience a spike in blood sugar levels due to chronic stress. Subsequent release of stress hormones (cortisol and catecholamines) disturbs glucose homeostasis and prevents muscles and white adipose tissue from absorbing glucose. This results in hyperglycemia, leaner body mass loss, visceral fat accumulation, and insulin resistance <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9561544\/#:~:text=Blood%20sugar%20levels%20may%20rise,regulation%20of%20these%20stress%20hormones.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">(Sharma K et al., Stress-Induced Diabetes: A Review. Cureus. 2022 Sep 13;14(9):e29142)<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h2><b>Role of Mental Health Support in Diabetes Care<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">A Fitterfly<\/span> <a href=\"https:\/\/diabetesjournals.org\/diabetes\/article\/71\/Supplement_1\/601-P\/145433\/601-P-Participation-in-Diabefly-Program-and\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\"> led by Neha Verma et al. (Diabetes 2022;71(Supplement_1):601-P) reveals that customised virtual stress management and behaviour therapy sessions helped manage diabetes distress among 100 T2DM (Type 2 Diabetes Mellitus) members who enrolled on Fitterfly Diabefly Program for 90 days. Their fasting blood sugar levels also significantly decreased by 25.00 from a baseline score of 142.00. They include psychosocial interventions like Intensive Behavior Therapy (IBT), Cognitive Behavior Therapy (CBT), Mindfulness-based Interventions, Gestalt Therapies, Motivational Enhancement Therapy (MET), Motivational Interviewing, and the COM-B Model of Behavior Change.
